    This man has polycythaemia vera , which is a myeloproliferative disorder leading to overproduction of red cells in the bone marrow. Patients sometimes get erythromelagia -a red or purple discolouration of digits and nails associated with swelling and burning pain.
    This man had far more obvious signs due to long standing poor distal perfusion from polycythaemia and acute cyanosis!
